The Gap
What exactly is the Issue?
People with several conditions see several specialists. Each prescribes for their own part of the picture, and the records rarely meet.
The result is hidden drug conflicts and side-effect cascades, such as pain medication that worsens cognitive decline. The carer who sees the patient every day has no way to put those observations into the clinical record.
